OFCCP Posts the Section 503 Self-Identification Form in Spanish and Word Formats
In response to requests from contractors, OFCCP just posted both the English and Spanish versions of the Voluntary Self-Identification of Disability form required by the new regulations implementing Section 503 of the Rehabilitation Act (Section 503) in .pdf and Word formats. The availability of these additional formats should make it easier for contractors to incorporate the form into their existing human resource information systems. Federal contractors are required to use this form to invite job applicants and employees to voluntarily self-identify as an individual with a disability as required by 41 CFR 60-741.42 of the new regulations. Consequently, applicants and employees should benefit from these additional formats as well. An English language version of the form was previously posted on the OFCCP Web site in portable document format (.pdf). The various versions of the form are available on the OFCCP’s Web site at http://www.dol.gov/ofccp/regs/compliance/section503.htm.
